SERVICE DETAIL

Client Support Services (Case Management)

The Intensive Case Management Program is specifically designed to provide vulnerable multi-barrier Government Assisted Refugees (GARs) and newcomers with a warm, welcoming, and safe environment along with essential support for a positive settlement experience in Canada during their first 12 to 24 months of resettlement. Our program adopts a systematic client-centered approach, which is focused on providing intensive case management services to assist in the settlement process.

Our program’s unique component is that our CSS case managers focus on providing services in the community, making home visits and community accompaniments. This strategy enables clients to build natural community connections where they live instead of receiving support in an office setting.

Our intervention is based on the client-centered, strengths-based, and collaborative approach, involving assessment of needs and assets, planning, linking, monitoring, and advocacy. During the assessment phase, case managers and clients work together to identify settlement needs, which may include physical health, mental health, housing situation, employment, education, navigation skills, environmental and weather challenges, language barriers, domestic violence, trauma, PTSD, and any other areas that may be affecting their quality of life and be an obstacle to their successful integration into the larger Canadian society.

Once needs are identified, our case managers work towards addressing them.
